The Short Answer: Go to My Team, toggle to next month, tap "Move" on a team, then tap "Here" where you want it to go. Changes are only allowed for future months, and they lock at 12:00 AM on the 1st.
You can only change your lineup for future months.

Once a month begins, its lineup is locked. You're always looking ahead.
Changes for next month must be made before 12:00 AM on the 1st.

Tap the My Team tab in the bottom navigation. You'll see your Starting 5 and Bench for the current month.
At the top of the screen, you'll see the current month with arrows on either side:
